Certification of Eligibility

I ________________________________ certify that my child (children) meet the eligibility requirements as set forth in the LCSAA Articles of Agreement and the CHA Athletic Board.  My child (children) are home schooled at least 51% of the time and do not actively attend another educational institution except as stated in the LCSAA Articles of Agreement.  Furthermore, I certify that my children are of the proper age to participate on the sports teams for which they desire to play.

Please fill out this form and bring to CHA Athletics Registration night or mail to Athletic  Director Billy Alexander.  If you are new to CHA Athletics, a copy of each child’s birth certificate is needed as well.
